Please ensure you have read this guidance before applying to join the BAPAM Directory.

Your progress will be saved if you go offline and return to the form later. However, we advise you to read through the questions in advance and have copies of the following documents ready to upload: 

1. Professional qualification and professional accreditation certificates
2. Current professional registration certificate
3. Up-to-date professional indemnity insurance certificate, covering all your health care work, including private, charitable, and NHS roles
4. DBS certificate issued within the last 3 years, or provide information we can check using the online DBS Update Service. Alternatively, we can arrange an enhanced DBS check for you if you cover the administration fee (currently £52 from DDC)
5. If your practice address is your own premises, your public liability insurance certificate
6. Evidence of CPD, relevant to providing health support in the performing arts
7. Your CV (optional – you can also provide employment details on the form)

Your application will be processed by a BAPAM administrator, an appropriate clinician and our Medical Director. We will contact you if we need any further information and, where possible, support you in meeting any additional requirements.
